Impact of inaccurate NHS patient records
Healthwatch England have heard from patients about frequent errors in individual medical records

Accurate patient medical records are crucial to delivering effective and efficient care. Healthcare professionals rely on medical records to support them in providing patients with the treatment they need.
However, 23% of people who were polled reported issues with their NHS records. Most of these inaccuracies relate to names, birth dates or, medication.
With plans to introduce a single patient record to the NHS, Healthwatch England look at what needs to be done to address “poor admin” and give patients more control over their medical history.
What needs to happen
- Better record keeping.
- Promotion of people’s rights to get records amended.
- Clearer guidance for patients about how to change incorrect records.
