New Healthwatch report about Hospital Discharge

We have published our latest report on hospital discharge. This is the findings from a discussion group we held with people from organisations involved in the hospital discharge process, including voluntary organisations. We talked about how patient discharge  hospital to being back home or being supported elsewhere is organised. Thinking about

  • the risk of problems
  • the involvement of patients and carers
  • how do we know the discharge process is working?

Healthwatch recommends further discussion of a number of points highlighted in the report and the involvement of patients and family carers in the development of improved information to help people understand what to expect and who to contact for what.

Effective discharge from hospital: the role of communication of home circumstances

This report looks at issues around preparation for hospital discharge and how understanding home circumstances is key for effective discharge arrangements for patients. Healthwatch Coventry makes 5 recommendations aimed at improving  practice. This piece of work is also aimed at stimulating dialogue and learning about what works.
